Not knowing more about what your goals are … I’d say just the cMap template.
If you want site visitors to be able to submit entries to your directory, maybe the Form extension too.
Or…
If you want business owners, who would need ot be registered users of your site, to be able to add and maintain their own entry, you would need the Link extension.

A colleague of ours suggested you to us and we were wanting to do something similar to Robin http://arizonamindbodyspirit.com/find-a-practitioner/ Which product is that using?
At this stage we would add and maintain the listings ourselves. Can that be changed in the future?
We do have gravity forms but would using your form extension be easier?

You need to have to check the “Notify me of follow-up replies via email” right below the “Tags” field before you submit the post. This is enabled by default, so you should get replies, unless you unchecked it.
