GuestOnly H2 headings show up. Anything beyond that like H3, H4 does not show up. Any idea?
I use a Genesis based theme.

I logged in and I was able to determine the cause of the bug and corrected it on your site.
Basically when creating the TOC the global settings options (the Settings : Table of Content admin page) were used for the when building the TOC item hierarchy instead of the headers set on the post settings.
In your case on the global settings you had only H2 enabled while on the post you had H2, H3 and H4 enabled. This caused only the H2 headers to be displayed in the TOC while leaving a blank space for the H3 and H4 headers.
The next update to ezTOC will include the fix I applied to you site so you will be able to update when it is released.
