Please open a new support topic in the WordPress support forums for Connections.
You need to have the
[connections] shortcode on the page. From what I can tell when looking at the page, you only have the widget shortcode to display the categories.

On the page with the categories, i did not use the shortcode. It´s pure handwriten HTML.
On the subpages, i use the connections-shortcode:
[connections category_in=9 order_by="SPECIFIED,zipcode"]
Ok, it that the HTML markup seemed identical to the category widget HTML. Regardless, for the /submit/ link to function on the homepage, you need to have the [connections] shortcode on the page. No shortcode, not form.
