ParticipantI have an issue with one of my contacts. The information is displayed properly but when you click on “View Profile” it returns no result.
The link generated is:
http://domainname.com/directory/name/first-lastname/
While the correct link should be:
http://domainname.com/directory/category1-directory/name/first-lastname/
It seems like it’s omitting the category in which the contact is created. Entry in question is OMalley.

No, that looks like the correct URL to me. The cat is not part of the URL because the entry can belong to an unlimited number of categories. You would have to share a link to your directory so I can take a look.
Make sure you have the directory home page set correctly on the Connections :: Settings admin page under the General tab.

What is the exact shortcode used on the directory home page and the specialty areas staff page?
Also, can you attach a screenshot of the admin edit page for OMalley.

Ok, but is she assigned to any of categories that have been excluded (including the children categories of the excluded categories).
That’s important because if she has been assigned to any of the excluded categories, she will not be able to be displayed.
Perhaps sharing a screenshot of the categories admin page will help… that way I can see the category IDs.
