Ok, the categories are there – thanx a lot!!
It just took 24 hours since my first try. :)
I am certain to have used your file. I downloaded and renamed it 3 times just to be sure. But nothing happened.
But maybe there is a hint for the next user with a similar problem. After I read of your success in my system I tried the plain TXT-file and got the mapping options!?!
So I went to the 2nd wordpress and tried one of my own files – renaming it to TXT – and it worked!
The mapping option appeared and the upload worked fine.
Please do not tell me, this is the usual way! :)

No, not at all. My guess you ran into a bug introduced in WP 4.7.2.
I suspect you would not be able to upload the .csv file without first renaming it to .txt into the WP Media Library either, being rejected as an unsecure file type.
I worked around this bug in the CSV Import addon. I may have to do the same for the Import Categories tool as well.
