    Site is http://www.Denverwellnessguide.com and I would like the submittable form option available in each category.  Is there a short code I need to implement, if so, what is the path and directions to do so please.  I have looked everywhere!

    I’m assuming you’ve purchased Form, yes?

    Here’s the FAQ that explains how to add Form to a page: How do I embed the form into a page?

    If you want it to show up on your directory page, just add it after the [connections] shortcode you already added.
