Do you have any other plugins that are for SEP or the page title? Does the theme have SEO or title options?
Can you install this plugin and past the system info here as a private reply?

GuestUpdate: It does seem to be an issue with the theme that I am using Care WP Version 3.0 By MNKY.
The problem disappears when I switch to WP 2014.
So knowing that, is there any way you can think of to remedy the problem?

Since it is definitely theme related, the best suggestion is to look around the theme’s options to see if there is any way to turn off their feature. If there is not, maybe the theme author can provide you with a filter or something that disables that feature … this does make me wonder if it interfering with All in One SEO too. These changes are better left to an SEO plugin and should not be built into a theme. The only reason I have these options at the moment is as a temporary stop-gap solution until I can fully integrate with Yoast’s WordPress SEO and maybe All in One SEO. My code will not interfere with SEO plugins because it only effects Connections pages which the seo plugin will not.
