It is there now. So I wonder what you did different from me. Do I need to save the entry somewhere? Because when I try it does not save. I go to add new custom field – I fill in the name. Do I have to do anything after that? Because it does not save when I do it.

yes, it is only saved when you actually save the entry in which you are attaching the custom field data to. This works much like the same as the identical fields when adding it to WordPress Pages and Posts.
Ahh I got it now. THen it shows up under the manage tab and my therapists could fill it in there.
