ParticipantSteven, my apologies – I already posted this in the FAQ for the Form add-on, but it looks like you want stuff here. Sorry about the duplicate post.
I’ve got the form plugin on our church website, and I can get the dropdown box for type of entry to display if I go into my Shape5 theme (maxed mag theme) and toggle a theme setting from “Display Raw HTML?” to “YES” – but then it applies it to the whole site. Using [raw] or [noformat] doesn’t work, so it appears that the theme setting is my only option. I can’t use that – I’ve got a coupe of people writing content for the church that won’t know how to correctly write the HTML. Turning it on for the whole site would be very painful! Is this something you’ve seen before?

Ok, now I see. The theme may have renamed [raw] or [noformat] to something else. Can I get a copy of the theme? You can post a private link to it here.

Unfortunately, no. The theme does not seem to be WP 3.6 ready, the theme admin fails to work. Likely because WP 3.6 uses jQuery 1.9. Since WP 3.6 is due out soon, you might want to keep that in mind and wait for thee theme author to verify compatibility.
Back to the issue on hand…
Even though the admin panel didn’t work, I was still able to view my Form text page and it did not exhibit this issue. So, now, I’m wondering if it is a plugin. Do you have Shortcode Ultimate installed?

It must have been a combination of the theme and TinyMCE Ultimate. I toggled the remove wpautop filter setting and everything seems alright now.

ParticipantHmm… you’re right, it worked, exactly the same as the S5 Theme setting to enable RAW HTML. My other pages all need fixed up now, since the <p> tags were removed.
I sure appreciate the help and time, Steven. I think it’s now up to my team and I to decide if we want to rework the rest of the pages w/out the wpautop filter so we can have a working Connections Form. I think it’ll be worth it – your suite of templates and add-ons are just too handy. :-)
