I have selected the fields I want to show up on my form entry page, but they aren’t exactly as I’ve specified and I hope you can help.

There are two things I want different on the form: 1. I don’t want the “Department” field under the Organization Name. 2. I would like the title for the Biographical Info field to match what it says in the directory listing, which is “Description.”
I think we’ve almost got it the way we want it. Thank you for your help.

That is part of the name fields which is a required fieldset. CSS can be used to hide it. Here is the CSS to hide the field:

Assuming you used the Template Customizer to change the label on the template, yes? To change the label in Form and in the admin, follow the instructions in this tutorial.
