    I created my own form and configured the fields that I wish to display. Nothing is displayed except for name or organization information – despite the fact that I have more options enabled as true.

    I also copied your demo code from the form on your demo page. This code exposes MORE than what is configured.

    so I am confused. Please help.

    You had the shortcode on the page more than once and the first shortcode had fancy quotes instead of regular single quotes. This is a WordPress requirement, not a bug in Connections. How this happens … from copy/pasting from a website into the text editor while in the visual tab. When working with shortcodes it is always best to manually type them in. I’ve fixed both issues. Hope that helps.
