I suspect the cause is something other than today’s update to Connections since the changes were limited nearly exclusively to the new WP Editor block for Connections. Also, the screenshot shows the Form addon which would have been unaffected to any changes in the core Connections plugin.
You’ll need to share a link to the page with the issue with me so I can take a closer look to determine exactly what the issue is. But, as a guess, I am thinking another plugin or he theme is breaking the styling of that element.
If you can not share a link with me, I suggest trying the conflict tests:

Create a temp account using this plugin:
https://wordpress.org/plugins/temporary-login-without-password/
Reply back, as a private reply, with the temp login link.

The conflict is with the WP File Manager plugin. It appears it might be interesting broken HTML into the WP Editor. Specifically its feature which adds the “Insert File Manager Shortcode” Button.
When this option is disabled in the WP File Manager Settings, the page shows like it should.
The option I disable can be found on the WP File Manager Settings admin page under the General tab at the bottom. The option is called “Enable “Insert File Manager Shortcode” Button with Content Editor ?”
Hope this helps!

ps.
RE: I hope this channel is confidential!
That is why I had instructed to use the plugin to create a temp login. That way user/pass are not saved in my forum. Using the plugin create self expiring links so they are of no value after they expire.
I removed the user/pass from your last post.
