This likely is being caused by a plugin conflict … but extremely difficult to say for sure. Can I get a temp admin account so I can see the issue firsthand. If that’s not possible, the only other thing that can be done is to deactivate all plugins except Connections and Form, then activate them one at a time until the issue returns. When it does, the last plugin activated is cause the issue. Please let me know what you find out.

Sorry, no, I did not look. I apologize as I missed your reply. I activated NextGen Gallery on my test site and Form still functions fine. So may there is a specific setting or perhaps an extension plugin that your using for NextGen Gallery which is the actual source of the problem.
You gave the user/pass … but not the login URL for the site in question so I don’t really know where to login and check.

Hmmm, that was the link I guessed at yesterday, from your profile. However, when I try to login, I get the invalid username/password error.

I still get the user/pass error. When I access the url directly I am redirected to here:
http://marion.crundwelldigital.com/login/
Maybe you have some IP filtering???
