I successfully .csv imported a list of entries, and can edit them fine. But when I go to add an New Entry manually, I can only add a name. All other fields have no entry fields showing. In my settings, I have one field for each checked. Also, when I uncheck Anniversary under dates, it always rechecks it. Please advise.
When I uploaded the .csv, I had to add Address to the end of the column name fields email and phone to get them to upload.
On the Fieldset Config admin page, make sure the repeatable option is on and/or you have the number of fields to display to 1 or more.

That worked, thanks. I just needed to enter 1 instead of 0 in the minimum number of fields box. (I should have read the text – I just assumed 0 = infinity like some other plugins.)
