Also, is there an easy way to pull a list of email addresses based on Category. The goal would be a custom template to select a category and receive a list of just the email field.
I haven’t done too much digging through the plugin yet, but is there a function provided to loop through entries and return email addresses based on a category?

Not done before, that I know of, not even by myself. Easy is relative, that would depend on how familiar you are with PHP and WordPress. IMHO, it should be somewhat painless and could probably be done in an afternoon.
I suggest taking a look at the code for the Hours extension. It’s a freebie so you’ll be able to download it.
It will not have an upload field but it does have pretty good inline code documentation. And it uses the same actions and filters you would need to added a new upload field.
In the action callback to save the file upload you would use the
$_FILESglobal and use the core WordPressmedia_handle_upload()to insert it into the media library. The returning value from the function will be the attachment ID which you would save as entry meta.re: is there an easy way to pull a list of email addresses based on Category. Sorry, no, there is not. But, you could use the Export All tool. This exports the data as a CSV file which you can load into a spreadsheet. From there you can filter by category to get the email addresses you want.

No, not really, but perhaps you could use this:
