After you install and activate the plugin. Activate your suport license.
Now, navigate to Appearance > Widgets page in the WordPress admin. Here you will find the Connections widgets along with all the other widgets that the theme and lother plugins have registered with WordPress. Click on the Connections Simple Search widget and then choose the sidebar in the theme to place the widget.
After you do that, the search widget will display live on your site.
I see you have broken the directory across many pages on your site — the search will only be applied to the page set as the directory home page on the Connections : Settings admin page under the General tab. I would highly suggest choosing a page which displays all the entries within your directory.
Hope this helps!

hard for me to say… displaying a widget is a function of the theme. Are you certain you place the widget in the correct sidebar? Does you site use some form of page caching? If it does try clearing it.
