ParticipantSure, ready to test, just deleted and uploaded the file you sent and the district field is now showing!
I’ll be adding some entries over the next few days and will let you know if I come across any bugs!

GuestSeems to be working in the back end (fields are showing and accepting input), however it seems that they are ignored by the short codes.
And I don’t know how to configure your forms add-on to use them?

How do you mean? There are no shortcodes for the fields. What are you attempting to do with the shortcode? Maybe I can add support in the next update.

There is nothing to do. Those fields will show up as part of the address fields; between the street address lines and the city/state line. The only reason this would not be the case is if you’re using an old version of Form (older than version 2.0). Which version of Form do you have installed?

I am trying to filter the results like you would for ‘city’ or ‘state’.
