When taking a look into the email logbook I see birthday mails are being send to the entry, but the notifications are not being send to the provided address.
In the log I can see that the From address is also being filled with the To address. Perhaps that’s the problem?

In the email log, does it say it was sent successfully? If it does, then there is not much I can do at that point. It is there that the email is handed off to the email server to send. WP basically just reports if that was successful or not. It is the job of the email server to send the email and that is beyond the control off WP and Connections.
If your web host provider has email logs, perhaps they can shed light on to it. If you are sending the notification to a Gmail address and the To/From are the same and the address is actually an email alias for another email address in Gmail then Gamil will ignore the email. I think… there is some combination of settings where Gmail will not receive an email.

Ok, so the email failing is the one being sent to the admin to notify the admin a birthday email was sent, correct?
The From address does not have a setting (in Connections or this addon). The address is pulled from the email address entered on the WP General Settings admin page.
If the address shown in the log does not match the email address found on the WP General Settings admin page then there must be another plugin installed which is overriding the address in that setting.

No, those “From” settings are for the email being sent to the person receiving the email not the admin notification email. The admin notification goes to the admin and the from address is pulled from the WP General Settings for the admin email address.09/25/2018 at 1:40 pm #473246
