Add the following CSS to the theme’s custom CSS area:

.cn-entry-content-block-list .cn-entry-content-block-meta > h3 {
    display: none;
}

That will hide the Custom Fields title.

Yes, this is possible, but you need to edit the template to do so. The file is card.php and it is found here:
../wp-content/plugins/connections-cmap/

The block of code you want to move is this:

<?php $entry->getContentBlock( $atts['content'], $atts, $template ); ?>
