My form installed well. I have two questions:
1) I have tested the form in two browsers, and the category dropdown is not working. Here’s the page: http://www.shuswapwomeninbusiness.com/swib-members/submit/
2) Is there a way to add the form to another page using shortcode?

It seems something is conflicting but I am not able to tell. Can you please try both the plugin conflict test and theme conflict test?

Are you planning on using Link to allow users to edit their own entries? If no, then yo can use the
[connections_form]shortcode. That said, I only officially support the submit link above the directory for now.Hope this helps, let me know the results of the conflict test.

I tested for theme conflict, and yes that appears to be the issue. Is there a workaround, please?

What theme is it? Is it a free them that I can download and test with? If not, you’ll have to give me a copy so I can work out the conflict, if possible.

Actually the theme has broken both drop downs. See that little button like thing next to the down arrow, that is the drop down that is supposed to be display for the template. It appears the them is being aggressive in formatting. It should leave the plugin functionality intact.
I’ll download the free version and see if they is a way to workaround these. I’ll get back to asap.

The free version of the theme does not exhibit this issue. Perhaps this is a feature in the premium version. Can you post, privately, a download link for the premium version for me to test against?
