We had a similar issue over a month ago and solved it, but it has appeared again, in a different form.
The first time this occurred we had to manually modify the cn_path location and provide appropriate permissions for the cache directory.
Not sure what has caused the problem to reappear, but this time I have discovered the Tinthumb error reported by others.
I have followed your suggestions in the post and have found this error in the chrome console for all the images (hundreds):
Failed to load resource: the server responded with a status of 400 (Bad Request) http://www.mammacare.com/wp-content/plugins/connections/includes/libraries/…wp-content/connection_images/Bradshaw_Dianah_original.jpg&h=130&w=100&zc=1
You had mentioned in the post that it is a permissions issue, but did not say how to remedy it.

Can you share a link to the directory so I can see the exact error messages? And so I can try other URL combinations to see if I can provide exact advise to fix this for you?
Something to try…
Change the permissions of the following folders and files to
767:
../wp-content/connection-images/
../wp-content/plugins/connections/cache/
../wp-content/plugins/connections/cache/index.htmlIf that does not do it, try:
Add the following to the
timthumb-config.phpfile found here:../wp-content/plugins/connections/includes/libraries/timthumb/This is what you need to add, make sure to change the doc root accordingly:
define ('LOCAL_FILE_BASE_DIRECTORY','/my/doc/root/');What
'/my/doc/root'will be, will depend entirely on the server setup. If you go to the the Connections : Dashboard admin page and look at the the System widget; there will be a constant named CN_PATH. The doc root should be the beginning portion of that path. What I can tell you is that from thewp-contenton will NOT be part of the doc root.Hope that helps!

The document root I applied was the same one when it worked before:

This issue is definitely the doc root…
Do not include

ParticipantThat definitely helped, once the thumbnails are cached, they will appear on refresh.
Is it possible to get the thumbnails to appear without being cached first? or requiring a refresh to render the cached thumbnails?
I’m concerned that visitors may not realize they have to refresh at least once before they see the thumbnails.

Not sure I understand … the user will not have to refresh to see the thumbnail. Maybe the browser cache needed to be updated after the doc root issue was sorted.
I visited every page within your directory and did not have to refresh once in order to view the thumbnail.
