Actually I think categories would be the best way to manage it. Use four parents with the options as children. Your site remind me of this one:

Yes, Connections does support custom fields. I’ve started documenting some of it. Take a look at the Tutorials sections on the Documentation Table of Contents page You would likely want to use a radio input for mail/email contact preference, so I would have to add a radio tuturial bu the process would be pretty close to the checkbox group custom field tutorial.

The reason is because the addons are still activated on the shopburnet.org site. You need to deactivate the licenses on the old domain and then activate them on the new domain. If you no longer have access to the old domain to deactivate them, go to your purchase history click the Manage Sites link and deactiovate the old domain for each license. If you need, I can also manually deactivate the licenses on the old domain, just let me know and I’ll do it.
After you do that then navigate to the Connections : Settings admin page and click the Licenses tab on the the new domain. Make sure you have entered in the support license keys and activated them. After you do you should be able to update without issue.

My first thought is that maybe the theme requires a lot of memory and it just so happens when Connections is activated, it pushes it over the limit and the result would be the same if you activate another larger plugin. So, that said, my first suggestion is to try increasing the memory allocated to PHP. Here’s an excellent post on how to do it. Since every web host is different, the method that will work for you will depend on the host server config. My suggestion is to start at the top and work your way down the list. Generally I recommend setting 128MB as that should, in most cases, provide plenty of headroom but try going straight to 256MB.
If changing the memory does not fix it, then I’ll need a copy of the theme for further debugging. If there ends up being something I need to address, I’ll fix it. But it could very well end up being an issue with the theme and not Connections.

Yes, not a template, but an extension. You’ll need at least the Link extension and might want, but not need, the Form extension.

Yes, you will need the Link extension (required) and will likely want the Form extension (optional).

The custom fields display block? The only way to move that would be to edit the PHP file for the template. If you are comfortable with PHP this is a relatively simply change.

Yes, see this FAQ for the details. The FAQ is for the cMap template but it works equally well with Tile Plus too.

Ok, I made the change you will now have access to the purchase history. When logging on you will need to reset your password using the forgot password link. The email address is the one you have been using to reply.
