The theme is doing something bad too, this code in the

Now back to the issue…
You can not select a time because of the Tribulant Newsletters plugin… it is also loading it scripts and CSS throughout the admin which it should not do.
It is loading an incompatible version of the datepicker script. Actually even worse is that they are enqueueing a version of the datepicker that comes with their plugin rather than the version that comes with WordPress. They really should be including the version that comes with WordPress. Well, it would not be so bad if they would enqueue it on only their admin pages. But the way it is now, any other plugin that uses the datepicker script will likely be broken due to this.
Unfortunately this is not something I can fix, you’ll have to contact them and ask them to fix it. Sorry.

I do not think you really want subscribers to access the admin so they can add/edit entries… but if you do, you also need to give them the View Admin Menu and View List (Manage) capabilities.
I’m thinking what you actually want is the Link extension where adding the add and edit entries capabilities allow the subscribers to add their own entries.

Just fired up my vm w/ Win7 and IE9… tested the page, its working fine for me…
After you change the department, in the URL do you see
cn-s=? And it is followed by anything other than&cn-cat=x(x would the the cat number)?Steven Zahm

Sorry no. I actually do not put a lot of dev time into this shortcode so it is rather basic. That said… the Widget Pack does come with a birthday widget which does link to the profile pages.

I assume you’ll be converting a single site WP install to a multi site install to do this??? OR two single site WP installs, just one sits on a subdomain???
I know converting a single site install of WP to multi site is doable, but troublesome. Never done it myself so I have no insight for you. Connections related… you’d probably would have to manually rename the tables and move the images to the correct folder so everything would “just work” again.
For two single installs where one sites on a subdomain. Shouldn’t have to do anything out of the ordinary when moving domains.

Thank you for the very detailed explaination, that helps tremendously! The part I was not getting was selecting a category should take you a different page and that destination page will depend on the category chosen from the drop down.
It is possible to change the destination page when selecting a category… but it is not possible to have multiple destination pages based on the category selected. Well, it is possible, but it would require some custom coding of javascript to change the form action based on the drop down choice. I do provide custom development solutions nut if you’d like to give a go yourself, here’s a Google search query with results that give examples of how to dynically change a forms action:

When you say title, you mean the person’s name? I recommend leaving that visible.
Here’s some CSS that’ll make it more compatible with your theme:
