It was your shortcode… there was all sorts of, uhm, odd invalid stuff within it which was breaking the WP shortcode parser so the real valid options were not being correctly read. I fixed it up the best I could attempting to decipher the intent of what existed in the shortcode. The address does display now and things appear to be the way you wanted them to be configured.

For some reason none of the table in Connections exists. Simply deactivating and reactivating Connections was the solution. Out of curiosity, how did you install and activate Connections?

Ok, if you are seeing the shortcode rather than the login form… are you certain you’ve installed the free Login extension?
