Are you certain you have purchased the Form extension? I see from your purchase record you have the Link extension, but not form. Perhaps you purchased Form another another account?

Ok, this is how I recommend setting this up. Instead of redirecting to the Form submission page, create a new “Member Homepage” (name does not matter).

On this page you can give details about their membership.

Also on this page you can use the Link shortcodes to embed links to the Form submission page.

And, optionally, setup the Link “Result List Actions”. These are the same links as the shortcodes but are presented to the user above the directory.

Of course you need to have Form activate on the Plugin admin page.

Is the intent to have registered WP users add and edit the own entry?

Let’s tackle one thing at a time….

Category 9 is Allergy & Immunology it has a single child category, Internal Medicine ID 10

If you set category 9 as the parent for the drop down, displaying Internal Medicine is the expected result.

To have a Specialties drop down, you should first create a Specialties category and then edit the existing categories so the parent is set to Specialties.

Then edit the search to be the ID of the Specialties category parent.

In short… never copy/paste stuff into the WP Visual editor from a webpage or email. Lots of unseen content also will be copy/pasted. Always switch to the Tab tab before you paste.

Basically you were seeing the shortcode, but it was not due to all the underlying content.

I corrected it. You can now set it up. I recommend making changes to shortcode while only in the WP Text tab.

The display issue is the result of the theme. It is applying styles globally to page nav elements and floating list items within the nav. Interestingly, from what I can tell, the theme is not using the nav for the menu, which is what would expect. It appears it is being used for social sharing link. But I was unable to determine under what conditions those are even displayed to the user. The theme support team would be better equipped to determine that.

As far the TOC not being auto inserted, that one is tougher. My guess it is the way the theme is interacting with the core WP the_content filter.

yes, but is complex. Here is an example:

Capitalization matters. Change Membership Type and Golden so it works with your data

The key is the Custom Fields Name and the value is the Value in the Custom Fields.
