Most have not logged in – thanks for explaining they won’t be linked until they log in.

I had LINK activated.

I did not activate “Submit new entry”

Solved – from Connections > Import CSV (NOT Tools)

Found the problem: Address Fieldset Repeatable > This was not checked AND minimum number of address fieldsets was set to 0. 0 = no address fields in the address field

No Joy – the addresses fieldset is blank within my form.

I turned off the address fieldset, unchecked each address field. Then I re-enabled each of them. Still no joy – address is a blank fieldset in my form.

Changed width to 200, but still the top of her head is cut off:

Image Size

The default size diameter of the image is 100px. This can be adjusted
with the image_width shortcode option. Here is an example shortcode
showing the image diameter being set to 200px.

Based on partial success with email fields, Contact form is now visible front end!

I turned everything (related that I could find) off and then on again.
In the backend:
Email fields are back and Contact Form is working in the front end!

BUT address fields are still missing.

Solution required 2 steps:
1 – I already required users to be logged in to see Form
2 – Settings > Display > Result List Actions > “Add My Directory Entry” link will be displayed.
