Ahh I got it now. THen it shows up under the manage tab and my therapists could fill it in there.

It is there now. So I wonder what you did different from me. Do I need to save the entry somewhere? Because when I try it does not save. I go to add new custom field – I fill in the name. Do I have to do anything after that? Because it does not save when I do it.

That looks very complicated. :( I will have to ask if someone can help me with it. I selected this plugin as it seemed easy to use.

Ok here goes. When I make a custom field under add an entry it shows up. [first attachement] but if I later return to add an entry the custom field I made is gone. So how do I save them? [see two attachments]

Ok so with those two extensions I can make a form that the therapists can fill in. I will give that a try then.

To clarify

I tried making a form under adding an entry but did not see a save button. The next time I opened adding an entry the custom fields were gone.

I am making directories for therapists and I would like for them to be able to add their own entry. So I thought a form would make that easier. Either something to be accesses online or through email.
