Hmmm… very likely this is being caused by the theme or another plugin “stomping” on the styles for Connections. I’ll need a link to the page with the issue so I can take a look and develop the CSS required to override and fix it for you.

re: My directory is only for a certain area within California. Is there a way to set up the form so that the person entering information has to verify their city? For example, making the City and Zip Code entry a requirement?

Sorry, no, this would have to be custom coded for you if that is a requirement.
