Thanks for letting me know! Seems like Yoast SEO broke the way they saved redirects. It no longer saves exactly what I want, rather it tries to correct what it thinks are errors (best guess).

I was redirecting /documentation/ to /documentation/contents/ just fine before I changed a permalink to a new FAQ and Yoast SEO auto added a redirect changing all the existing one in the process.
