Unless a multiple site license is purchased (only available on the bundles presently) the license is valid for a single site.
Your Form license is activate on centralcareers.org. If you are activating it on different site, you should purchase a second license or a bundle which include multiple site license options.

The license is not disabled. It is current and active and does not expire until June of next year.

There is only the license key which includes support and updates while it is active. The license is valid and active for a year from date of purchase.
The info about this is on the Form product page. It is located on the right hand side under the plugin info. The info also includes a link which includes more specific details. These are the same terms and conditions as agreed to when making the purchase.
RE: in the backend, it still says my license is not activated, but the form suddenly works now…maybe that was the support license key that expired.
An unactivated license or expired license will not prevent the plugin from functioning. You will however stop receiving support and updates with an expired license. This is how nearly all plugin licenses work for commercial plugins. It is almost the standard which is why I do not deviate from it as it is generally very familiar to those who purchase commercial plugins.
