Oh, I see you have two license for Form. I suspect you are trying to activate the same license on two different sites:

  1. http://richelais.eu/blogducardinal/
  2. http://richelieu.eu/

You need to activate one license on one site and the other license on the other site. You can not activate the same license on both sites.
