It seems the Default Card template is being used. It is manually being set by the shortcode option template='card' . The image should show but it appears that the template may have been altered or perhaps the Slim Plus template files are being used as override files for the Default Card template. You would have had to move files around on the server to do that, so you could undo it.

My suggestion, remove template='card' from the shortcode. Ensure the single entry template option is disabled. Verify that the Slim Plus template is the active template on the Connections : Templates admin page.
