That is a WordPress error. Generally those are triggered when a security feature called an nonce has expired.This can be remediated by refreshing the page and trying do the action action again. These mainly popup for me when WP decides to expire my login session. So I have to login again and redo my action.

Yes, but there is no user interface to add those fields. See this tutorial on how to add custom text fields.
