I am unable to view the form because it requires a login. You can post login info here as a private reply so I can take a look.

What I did notice on the page was a couple fatal javascript errors. These could be the cause. You can view the errors by loading the page in Chrome, hitting the F12 key and then clicking the Console tab. Be a little patient for the second error to show since the site is a tad slow to completely load the page.

Here are the errors:

  • Failed to load resource: the server responded with a status of 500 (Internal Server Error)
    http://aussietaste.recipes/wp-admin/admin-ajax.php
  • Failed to load resource: the server responded with a status of 504 (Timeout while reading response from Server)

It seems the server is blocking access to a core WordPress file.

My suggestion is to first resolve these two errors to see if the bio box issue is resolved too.
