Ok, I logged in and installed the Query Monitor plugin. The table for both the addresses and phone numbers were being reported as “crashed and should be repaired“. With that information I installed the WP-DBManager plugin and used it to repair those two tables.
That seems to have fixed the issue as I was able to add a entry with an address and phone and then edit them.
I spot checked a few other entries and the addresses and phone numbers were available to edit.
If any addresses or phone numbers were added after the tables became corrupt, you will very likely have to add them again when editing the entry since that data could not be permanently saved. These two tables have quite a bit of data saved so I assume most of your data is safe.
What would cause this? Sorry, but I do not know and I have never seen this before. It is completely possible that you may have other tables that are corrupt too but I do not think so because I think WP-DBManager would report it on its overview page. But I am not certain of that.
I’ve left both the Query Monitor and WP-DBManager plugins installed. You can safely delete these.
