The shortcode for the Form extension is [connections_form] however this shortcode is deprecated and no longer supported which is why you will find no documentation on it. It is only provided as backward compatibility for those still using it. What you should do instead is go to the Connections : Settings admin page, click the Display tab. Under the Results List Actions section you can enable the link which will add the “Submit New Entry” link as explained on the installation page.
