For the second part of your question that was answered to me: “Please log out of your site and then test a submission thru Form … all submissions will be sent to the moderation queue. When you submit an entry while logged in, you are adding an entry with the same permissions as if you were submitting a new entry using the admin.” You have to have the form checked to require login for this to happen
