In short, I think the cMap template is what you need. It meets nearly all your requirements. You can make it look more like the default template by removing its gradient, see this doc page for more.

The one item that is not met is the category links. Right now, that is not possible regardless of the template. Oh, cMap actually does not show the categories the entry has been assigned to like the default template does.
