The disappearing image was due to IE running in Compatibility View. I removed the whiterockzone.com site in the list for sites to view in compatibility view.

If anyone else has these problems, this is worth a try.

The Visual editor tab is still an issue in both IE and Firefox. Your next update may fix it.
