The CSV Import extension can be downloaded from your purchase history with an active license. The files for the extension in your archive can be found in this path: ../wp-content/plugins/connections-csv/

The category data is actually in the WordPress database. It is not stored in a file. If you still have the database from the old site, you’ll have to use phpMyAdmin or similar tool and export the Connections tables and reimport them into the new database.
