You have the following page set as the home page:

http://birthingfromwithin.com.158-69-37-116.vanilla.websavers.ca/directory/

The directory home page will need to be set to a page which has the [connections] shortcode on it.

The Form settings on the Connections : Settings admin page has options which you can use to place content before and after the form.
