Reply To: Entry Form Address Field Names

Hi Steven,
I have tried practically every form of entry that I could think of with no luck. As you have carried out this specific action yourself could you please give me exactly what you would expect to enter to the four fields that are presented when Add New is selected. I am sorry for my apparent inability here but this is a new area for me.
