Perhaps you are using some sort of page caching, either with a WordPress plugin or on the server? If you, after changes to WordPress you should flush the cache.

re: the choice in admin panel is limited. I can remove index.php only in the “personnal structure” but if I do that all the links are broken

Navigate to the Connections : Tools admin page and click the System Info tab. Scroll to the bottom and generate a Remote Viewing link and share that with me please.

Sorry, no, you can not create custom fields using the Form extension. Custom fields have to be coded.
