Try this instead:

[upcoming_list list_type='membership' show_lastname='true' list_title='Rotary membership anniversaries in the next 30 days' template='anniversary-light']

I forgot the template slugs are actually anniversary-light, anniversary-dark, birthday-light and birthday-dark which explains the template not found notice. Apologies!
