It seems you license for Form has been activated on thriveincanada.ca. Support licenses are only good for a single site. To activate on you will first have to deactivate it on thriveincanada.ca. I see it is installed on that site but it does not actually appear you are using it on that site so I would suggest deactivating the license and then the plugin on that site.
