Seems to be an issue in the theme, not Connections or Form. The theme is overriding the version of jQuery included with WordPress, loading a very old version. Here are a couple suggestions:

  1. See if the theme dev offers an update for your theme which corrects this issue.
  2. Navigate to the Connection : Settings admin and and click the Advanced tab. Scroll down to the Compatibility section and enable the jQuery compatibility option. This will attempt to fix loading the correct version of jQuery.
