Add str_select_all='' to your shortcode. That will remove the “Show All Categories” option.

Removing the “Test Centers” at the top will require a bit more work. Instead of setting the category shortcode option to the category ID of “Test Centers” you will have to use the ID for each country instead. Hope that makes sense.
