They are using one of the core templates that come with Connections. It is the Bio Entry Card. I don’t think they made any modifications to it.

If you need some custom work on a template, I am not available, but I do have a developed that I can recommend. He has plenty of experience with working with Connections. Just let me know, and I’ll ask him to reply here so you two can make contact.
