I wouldn’t have sent a new request if it wasn’t different :-)

The first post was about how to view the form at all

This post was about how to display the fields I want

It’s not really resolved – I cannot understand how to get the code right for what I want

I’ve gone over and over changing this and changing that and it won’t display how I’d like it to. I’m not technical so this is really frustrating after paying out so much money
