If you wish to accept entry submission from site visitors, not registered site users, you would use Form. Link is to allow registered site users to submit and maintain entries. So it you’re accepting only from users, I recommend Link rather than Form. Form will at some point integrate with Link to allow from end user editing of one’s entry. No ETA on that other than next year.

As for limiting the selecting of the address/phone type. Not easily doable. In the 0.8 release of Connections I will have a filter that can be hooked into to limit the displayed options but that still would take a bit of minor PHP dev work.
